To volunteer or to continue to be employed at St. Catherine Laboure’, the following information MUST be completed and returned to the Parish Business office. These have been consolidated into two documents - links below.
1. Family Care Registry
This is the new background check form that must be filled out by all volunteers/ employees.
If you are 18 years old or older, the Missouri Department of Health and Senior Services requires that you fill out the "Worker Registration" form. This form allows our office to run a background check on you. A completed form with a copy of your social security card is required. SCL will pay for the record check, so the completed form and copy of your social security card should be returned to us. If you have filled this form for another Agency or Parish for the Archdiocese, we will only need your social security number to do the background check.
All adults who work with children and teens for the Archdiocese of St. Louis, either as paid or volunteer workers, must complete this form as part of the overall policy of our Archdiocese. Please click at the bottom of this page for the form.
If you do not wish to give us a copy of your Social Security card, you may go online to www.dhss.mo.gov/FCSR/BackgroundMSHP.html and complete the information, print your receipt and return with all paperwork to us.
2. Protecting God’s Children Workshop
In addition, all volunteers 18 years or older must attend a Protecting God's Children Workshop given by the Archdiocese of St. Louis before working with students. For a schedule of upcoming workshops CLICK HERE. Volunteers should attend these.
If you have already attended a workshop, you are not required to go twice.
*If you will turn 18 years old during the school year, it is advisable that you attend a workshop over the summer prior to your year of service, since when you turn 18 it is a requirement.
Code of Ethical Conduct
Each person 18 and older must read and sign the "Code of Ethical Conduct" for the Archdiocese of St. Louis. Click at the bottom of this page of this document.
The Archdiocese is now mandating that every volunteer sign an acknowledgement that we will be running a Background Check. This must be turned in with the items listed above.
